Transition (Young adults)

On leaving school, individuals with a disability need to plan and initiate positive adult lives with the support of their families.

Transition supports and encourages the Transition of young adults with a disability as they leave behind the role of student and begin to take on adult roles.

Transition has a dual focus as it provides support not only to the individual but also to their families.

Eligibility

Individuals who have received funding through the Post School Services - Adult Lifestyle Support Program and who participate in Building Informal Networks.

How does it work?

The Key Worker is responsible for the day-to-day management of the Program and reports to the Team Leader.

Transition provides a Key Worker who will assist the individual to:

  1. experience a range of opportunities as they become an adult;
  2. change as their role within the family changes, in keeping with their developing adult status;
  3. participate increasingly in the life of their community;
  4. build on existing relationships and networks; and
  5. explore a range of recreational, educational and work opportunities.

Mamre asks individuals and their families to;

  1. support the purpose of Transition;
  2. meet accountability requirements for cash transfers (See Policy and Procedures); and
  3. give constructive feedback on Mamre's services.

If a family or group approaches Mamre for support in establishing their own adult life-style service, Mamre will consider its capacity at the time to provide the required support for an agreed limited time.

Within this capacity, Mamre will assist the family or group to:

  • clarify their mission and values;
  • identify the best service model which suits them;
  • apply for any funding that may be required; and
  • connect with similar initiatives.
